Supporting materials comprise one part of the online application. To assist you in planning ahead, required templates can be downloaded for completion.  A complete list of the required supporting materials and instructions for their preparation are provided on this site.

This page contains the downloadable templates for three of the supporting materials that the Fund requires the applicant to complete as a part of the application.  The templates may not be altered; additional pages may not be added to the templates unless the instructions indicate otherwise. Either the Word or PDF version of the templates may be used to prepare the documents, with the following stipulations:

• MS Word Templates: Word templates must be completed in the spaces provided and must respect the original margins on all sides.  You must correct any changes to the template resulting from input of data. MS Word files must be converted to Portable Document Format files.
• PDF Templates: Adobe Acrobat may be used to complete and save the PDF templates to your system. Reader and Mac's Preview are not compatible for saving and transmitting the information.  Portable Document Format (PDF) is the only acceptable format for attachments to the grant submission website.

The templates must be:

• Prepared according to the instructions in the RFP (request for proposal), converted to, or formatted as Portable Document Format (PDF) documents, and combined with the other supporting materials.

You will later upload the completed combined file as an attachment to your online application.
